Please read the following Terms of Use (the “Terms”) carefully before using any website of Core Value Insurance Group LLC  (“CVIG”, “our”, or “we”) and its affiliates, including, but not limited to, the website at www.cvig.com, as well as any mobile applications, communications features, online features or applications, services and/or programs offered by CVIG (collectively, the “Web Properties”).  

By accessing or using the Web Properties, you agree to the following Terms.  If you do not agree to any portion of these Terms, you should not access or otherwise use the Web Properties.  

You should review these Terms regularly as they may change at any time in the sole discretion of CVIG.  As used in these Terms, “Content” refers to any data, comments, suggestions, feedback, information, text, software, sounds, photographs, audio, audiovisual, video, documents, images, logos, artwork, graphics, designs, messages and other materials of any nature. 

  1.  CONVENIENCE AND INFORMATION ONLY.  The Web Properties are provided to you as a convenience and for your information only.  By providing access to the Web Properties, CVIG does not represent or warrant that:  (a) the Content is accurate or complete; (b) the Content is up-to-date or current; (c) CVIG has any obligation to update any Content; (d) the Content is free from technical inaccuracies or typographical errors; (e) the Content is free from changes caused by a third party; (f) your access to the Web Properties will be free from interruptions, errors, computer viruses or other harmful components; and/or (g) any information obtained in response to questions asked through the Web Properties is or will be accurate or complete. 
  2.  CVIG’S ROLE IN PROVIDING THE WEB PROPERTIES.  
  • (a)  The services described in the Web Properties are provided by CVIG.  In this capacity, we act as an insurance broker or agent.   CVIG and its representatives are appropriately licensed in all states in which they conduct business.  CVIG and its representatives are certified representatives of Medicare Advantage (HMO, PPO and PFFS) organizations (and stand-alone prescription drug plans) with a Medicare contract. CVIG is not an insurance carrier. Certain insurance products/services may be recommended to you, and all products/services are provided by third-party insurance companies (collectively “Carriers”) which are unaffiliated with CVIG.  CVIG does not issue insurance contracts or process claims.
  • (b)  The decision to enroll in any product/service is made by you. The Carrier is solely responsible for its products/services to you.  No insurance product or service is offered or will be sold in any jurisdiction in which that offer, solicitation, purchase or sale would be unlawful under the insurance or other laws of that jurisdiction. You should understand that the Carriers may rely on the statements and representations that you make during the underwriting process. The Carriers may reject an application if there is a suspicion of fraud or for other reasons.  
  • (c)  CVIG earns a commission paid by the Carrier based on your enrollment in a health plan.  CVIG’s representatives are not compensated based on your enrollment in a health plan and do not receive commissions from third-party insurance companies.  The Web Properties may be considered advertisements or solicitations for insurance.  
  • (d)  By using our services, you understand, acknowledge and agree that: (i) nothing in these Terms or in the Web Properties constitutes an offer or guarantee of coverage; (ii) requirements for a specific health insurance product or service are made by the Carrier of that product or service; (iii) CVIG does not guarantee any Carrier’s product or service; and (iv) CVIG is not and will not be liable for any damages, costs, liabilities or losses of any kind arising out of or in connection with your use of any Carrier’s product or service. 
  1.  USING THE WEB PROPERTIES AND SERVICES.  
  • (a)  Users of the Web Properties may view, copy or print pages from the Web Properties solely for their own personal purposes.  Users may not otherwise use, modify, copy, print, display, reproduce, distribute or publish any information from the Web Properties without the express, prior, written consent of CVIG, nor may users, reverse engineer, modify, or create derivative works from any content or software from the Web Properties without our prior express written permission.  At any time, CVIG may, without further notice, make changes to the Web Properties, to these Terms and/or to the services available through the Web Properties.  We will make an effort to update this page with any changes to these Terms and/or to the services described in these Terms.  
  • (b)  Although the Web Properties contain descriptions about health insurance products/services, those descriptions are informational only and are not intended to include all terms and conditions. All coverage is subject to the terms and conditions in the insurance policies and documents issued by the Carriers. 
  • (c)  While our decision support tool in the Web Properties may rank certain health insurance options in order of “Plan Premium (Low to High)”, “Total Estimated Annual Cost”, “Medicare Star Ratings”, or “Plan Name (A-Z)” based on your answers, you are responsible for ensuring that the coverage you select is appropriate for you and your dependents, and that the information you provide is accurate and current. The costs for coverage shown in the Web Properties are estimates that may vary. 
  • (d)  Your use of the Web Properties does not guarantee that you are eligible to receive coverage under any insurance product/service.  To enroll in any insurance product/service, you must be eligible for the particular product/service and you must complete the enrollment process. There may be errors in the information displayed in the Web Properties or the information may not be complete or current and it may not apply to your situation.  
  1.  ACCESS.  You must provide all end-user equipment, software and telecommunications services necessary for you to access and interact with the Web Properties and to receive email and/or text communications from us related to your activity with the Web Properties. We are not responsible for any fees, costs or expenses incurred by you in connection with the foregoing or your use of the Web Properties (e.g., internet provider data charges, mobile phone data charges, etc.). At certain times, access to the Web Properties may not be available due to maintenance or circumstances beyond our control. We may discontinue or modify access to the Web Properties at any time without prior notice. 
  1.  INSURANCE DISCLAIMER.  
  • (a)   We do not offer every plan available in your area. Any information that we provide is limited to those plans that we do offer in your area.  Please contact www.medicare.gov or 1-800-MEDICARE (TTY users should call 1-877-486-2048), 24 hours a day/7 days a week to get information on all of your options. 
  • (b)  ATTENTION: Medicare has neither reviewed nor endorsed the information contained in the Web Properties.  CVIG is not connected with or endorsed by the U.S. government, the FDA, or the Centers for Medicare & Medicaid Services. 
  1.  NO WARRANTIES; DISCLAIMER OF LIABILITY.  YOU EXPRESSLY AGREE THAT YOUR USE OF THE WEB PROPERTIES IS AT YOUR SOLE RISK.  THE WEB PROPERTIES AND ALL CONTENT IS PROVIDED “AS IS” AND “AS AVAILABLE” FOR YOUR USE, WITHOUT WARRANTIES OF ANY KIND, EITHER EXPRESS OR IMPLIED, UNLESS SUCH WARRANTIES ARE LEGALLY INCAPABLE OF EXCLUSION.  CVIG MAKES NO REPRESENTATIONS OR WARRANTIES THAT THE WEB PROPERTIES, ANY CONTENT OR ANY SERVICES OFFERED IN CONNECTION WITH THE WEB PROPERTIES ARE OR WILL REMAIN UNINTERRUPTED OR ERROR-FREE, THAT DEFECTS WILL BE CORRECTED, OR THAT THE SERVICES AVAILABLE ON, THROUGH  OR IN CONNECTION WITH THE WEB PROPERTIES OR THE SERVERS USED IN CONNECTION WITH THE WEB PROPERTIES ARE OR WILL REMAIN FREE FROM ANY VIRUSES, WORMS, TIME BOMBS, DROP DEAD DEVICES, TROJAN HORSES OR OTHER HARMFUL COMPONENTS.  CVIG DOES NOT GUARANTEE THAT YOU WILL BE ABLE TO ACCESS OR USE THE WEB PROPERTIES AT TIMES OR LOCATIONS OF YOUR CHOOSING, OR THAT CVIG WILL HAVE ADEQUATE CAPACITY FOR THE WEB PROPERTIES AS A WHOLE OR IN ANY SPECIFIC GEOGRAPHIC AREA.  
  2.  LIMITATION OF LIABILITY.   IN NO EVENT WILL CVIG BE LIABLE FOR ANY DIRECT, INDIRECT, INCIDENTAL, SPECIAL, EXEMPLARY, PUNITIVE OR CONSEQUENTIAL DAMAGES ARISING FROM YOUR USE OF THE WEB PROPERTIES, ANY CONTENT AND/OR ANY PRODUCTS OR SERVICES PROVIDED ON, THROUGH  OR IN CONNECTION WITH THE WEB PROPERTIES OR FOR ANY OTHER CLAIM RELATED IN ANY WAY TO YOUR USE OF (OR INABILITY TO USE) THE WEB PROPERTIES, ANY CONTENT AND/OR ANY PRODUCTS OR SERVICES PROVIDED ON, THROUGH  OR IN CONNECTION WITH THE WEB PROPERTIES.  BECAUSE SOME STATES OR JURISDICTIONS DO NOT ALLOW THE EXCLUSION OR LIMITATION OF LIABILITY FOR CONSEQUENTIAL OR INCIDENTAL DAMAGES, IN THOSE STATES AND JURISDICTIONS, CVIG’S LIABILITY WILL BE LIMITED TO THE GREATEST EXTENT PERMITTED BY APPLICABLE LAW.  
  3.  INDEMNIFICATION.  You agree to defend, indemnify and hold harmless CVIG and its directors, officers, employees, agents and representatives from and against any and all claims, demands, suits, proceedings, liabilities, judgments, losses, damages, expenses and costs (including, but not limited to, reasonable attorneys’ fees) assessed or incurred by CVIG, directly or indirectly, with respect to or arising out of:  (a) your failure to comply with these Terms; (b) your breach of your obligations under these Terms; and/or (c) your use of the rights granted hereunder, including, but not limited to, any claims made by any third parties.
  4.  PRIVACY.  Personal data that you provide regarding yourself will be handled in accordance with CVIG’s Privacy Policy [INSERT LINK TO PRIVACY POLICY]. Children under the age of thirteen (13) are prohibited from submitting any personally identifiable information.  If we learn that a user under the age of thirteen (13) is publicly posting or sharing personally identifiable information on or through the Web Properties, then we will use commercially reasonable efforts to block that user from using the Web Properties. 
  5.  ELECTRONIC COMMUNICATIONS.  By using the Web Properties, you consent to receive electronic communications from us unless you follow applicable opt out procedures. We will communicate with you by email or by posting notices on the Web Properties. You agree that all agreements, notices, disclosures and other communications that we provide to you electronically satisfy any legal requirement that such communication be in writing.  
  6.  LINKS TO THIRD PARTY SITES.  CVIG may offer health and wellness products and services to help consumers better manage their health and the payment of healthcare. These products and services are offered through links from the Web Properties to websites that are owned or operated by the Carriers or other third parties unaffiliated with CVIG (each, a “Linked Third-Party Site”). THE LINKS TO LINKED THIRD-PARTY SITES ARE PROVIDED FOR YOUR CONVENIENCE AND INFORMATION ONLY.  These links do not constitute or imply CVIG’s approval, sponsorship or endorsement of any Linked Third-Party Site. CVIG is not responsible for and makes no representations or warranties, express or implied, regarding any Linked Third-Party Site.  When you access and use a Linked Third-Party Site, you are subject to that third party’s terms and conditions of use. Any rights, claims or actions you may have in respect of a Linked Third-Party Site can only be brought directly against the owner or operator of the Linked Third-Party Site. IF YOU DECIDE TO ACCESS ANY OF THE THIRD PARTY WEBSITES LINKED ON THE WEB PROPERTIES, YOU DO SO ENTIRELY AT YOUR OWN RISK.
  7.  THIRD PARTY CONTENT. The Web Properties may include articles, opinions, and news on personal health and financial health. Some of this content is supplied by individuals and companies that are not affiliated with CVIG (“Third Party Content”). Some Third Party Content may be provided by companies whose products and services are offered through the Web Properties.  We will use our best efforts to clearly and prominently identify the source of all Third Party Content.  Third Party Content is protected by copyright under United States laws and international treaties and is owned or licensed by the Third Party Content provider(s) credited.  CVIG has not been involved in the preparation of Third Party Content and does not explicitly or implicitly endorse or approve any such content.  While CVIG makes every attempt to provide accurate and timely information to serve the needs of users, neither CVIG nor any Third Party Content provider guarantee its accuracy, timeliness, completeness or usefulness, and are not responsible or liable for any such content, including any advertising, products, or other materials on or available from third party sites. Third Party Content is provided for informational purposes only and CVIG specifically disclaims any responsibility for Third Party Content available through the Web Properties.  You agree to use Third Party Content only at your own risk.  All Third Party Content is provided on an “as-is”, “as available” basis.  
  8.  PATENTS, COPYRIGHT AND TRADEMARKS.  CVIG is the copyright owner or authorized licensee of all text and all graphics that CVIG posts on or in the Web Properties.  All trademarks and service marks of CVIG, or its subsidiaries or affiliates, that may appear or be referred to in the Web Properties are the property of CVIG, or one of its subsidiaries or affiliates.  Other parties’ trademarks and service marks that may be referred to in the Web Properties are the property of their respective owners.  Nothing in the Web Properties should be construed as granting, by implication, estoppel or otherwise, any license or right to use any of CVIG’s, or its subsidiaries’ or affiliates’, patents, trademarks, service marks or copyrights without CVIG’s prior written permission.  CVIG aggressively enforces its intellectual property rights.  Neither the name of CVIG, its subsidiaries or affiliates, nor any of CVIG’s other trademarks, service marks or copyrighted materials may be used in any way, including in any advertising, hyperlink, publicity or promotional materials of any kind, whether relating to the Web Properties or otherwise, without CVIG’s prior written permission.  
  9.  AVAILABILITY.  Information that CVIG publishes in the Web Properties may contain references or cross-references to products, programs or services of CVIG, its partners, subsidiaries or affiliates, that are not necessarily announced or available in your area.  Such references do not mean that CVIG, or any of its partners, subsidiaries or affiliates, will announce any of those products, programs or services in your area at any time in the future.  You should contact CVIG for information regarding the products, programs and services that may be available to you, if any. 
  10.  TERMINATION OF SERVICE.  We may terminate your ability to use or access secured portions of the Web Properties at any time, without notice, for conduct that we believe violates these Terms and/or is harmful to other users of the Web Properties, to CVIG, to its partners, to CVIG’s service provider, or to other information providers.  
  11.  GOVERNING LAW; VENUE. These Terms will be governed by the laws of the Commonwealth of Pennsylvania, without giving effect to any principles of conflicts of laws.  By using or accessing the Web Properties, you agree that any action at law or in equity arising out of or relating to your use of the Web Properties or these Terms will be filed only in the state or federal courts sitting in Philadelphia, Pennsylvania, and you hereby consent and submit to the personal jurisdiction of such courts for the purpose of litigating any such action. 
  12.  LOCAL LAWS.  CVIG makes no representation that content or materials on the Web Properties are appropriate or available for use in jurisdictions outside the United States.  Access to the Web Properties from jurisdictions where such access is illegal is prohibited.  If you choose to access the Web Properties from other jurisdictions, you do so on your own initiative and are responsible for compliance with applicable local laws.  CVIG is not responsible for any violation of law.  You may not use or export the Content or materials in the Web Properties in violation of U.S. export laws and regulations.  You agree to comply with all applicable laws regarding the transmission of technical data exported from the United States and the country in which you reside (if different from the United States). 
  13.  CUSTOMER COMMENTS.  We welcome the submission of comments, information or feedback through the Web Properties.  By submitting information through the Web Properties, you agree that the information submitted will be subject to these Terms and CVIG’s Privacy Policy

Your Consent To These Terms of Use 

By accessing and using the Web Properties, you consent to and agree to be bound by the foregoing Terms.  If we decide to change these Terms, we will make an effort to post those changes on this page so that you will always be able to understand the terms and conditions that apply to your use of the Web Properties.
