- Request a Callback via the Online Form. If you request to speak to a licensed agent, you will need to fill out the online form, which requires your full name, telephone number, email address, date of birth, state, and zip code. We use this information to communicate with your regarding your Medicare questions, provide support, or help you with a new Medicare plan.
- Communicate with Us. You may need to contact CVIG, but not a licensed agent. You can do this by email, telephone, or by stopping by our office. If you contact us by email, we will collect your email address and any other information, such as name or telephone number, you provide in the body of the email. We will never request, nor do we accept, your medical records, please do not provide them. If you contact us by telephone, we will collect your name, telephone number, and any other information related to your inquiry. Our Website uses Google Maps to provide you directions to our office. Once you click on the “Find us on Google Maps” button, you will automatically be redirected to Google Maps and be subject to Google’s Privacy Practices and Terms and Conditions. - Interact with Us on Social Media. When you interact with us on social networking websites, such as Meta, TikTok or LinkedIn, we collect basic engagement metrics and use it to tailor content and marketing and use it to improve user experience. Because our social media pages are publicly accessible, when you use them to interact with other users, for example by posting, leaving comments, liking, or sharing posts, any Personal Information you provide can be viewed by others or used by them as they see fit. The content your post on our pages can be deleted in the same way as other content you have created. We do not control the use or storage of information that you have posted to any social networking websites. This information is collected and processed by the social networking websites for their own purposes, including marketing. - Apply for a job. If you apply for a job, you will be redirected to our Careers website. You will provide us your name, email address, telephone number, physical address, resume, cover letter, and references. We use this information to communicate with you regarding the position, review your background and skills, and assess your qualifications.
- Aggregate data. We aggregate the data we collect for benchmarking purposes and for internal analytics.

General Sharing
CVIG shares Personal Information in the following instances:
- Within CVIG. We share your Personal Information among our affiliated entities for the legitimate business purposes of efficiently and effectively providing bonds to our customers. Access to your Personal Information is limited to those on a need-to-know basis.
- In the event of a corporate reorganization. In the event that we enter into, or intend to enter into, a transaction that alters the structure of our business, such as a reorganization, merger, acquisition, sale, joint venture, assignment, consolidation, transfer, change of control, or other disposition of all or any portion of our business, assets or stock, we would share Personal Information with third parties, including the buyer or target (and their agents and advisors) for the purpose of facilitating and completing the transaction. HIPAA NOTICE OF PRIVACY PRACTICES
The Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) Privacy Rule allows individuals who provide protected health information to covered entities or business associate the right to receive a notice that describes how their protected health information may be used and/or disclosed and how to acquire access to this information. CVIG will never ask or accept medical records from you. However, you may provide certain healthcare information about yourself in order for us to assist you in applying for insurance coverage. We will only use your healthcare information for that purpose and no other purpose. We do not provide your healthcare information to third parties, except for the health insurance providers (or their agents) to whom you are applying.
DO NOT TRACK
We do not respond to Do Not Track requests. Do Not Track is a preference you can set in your web browser to inform websites and mobile applications that you do not want to be tracked. You can enable or disable Do Not Track by visiting the Preferences or Settings page of your web browser.
INFORMATION SECURITY
We implement and maintain reasonable security measures, such as access controls and encryption, to protect the Personal Information that we collect and maintain from unauthorized access, destruction, use, modification, or disclosure. CHILDREN’S PRIVACY
Our Website is not directed to or intended for children. We do not knowingly collect or post information from children under the age of 13 in compliance with laws protecting the privacy of children.
