This Privacy Notice describes the information collection, use, and sharing practices of Core Value Insurance Group, LLC (“CVIG”, “us”, “we”, “our”) when you interact with us through our website or any electronic means (collectively the “Website”).

PERSONAL INFORMATION WE COLLECT DIRECTLY FROM YOU 

We collect personal identifiable information (“Personal Information”), which is information that identifies, relates to, describes, is capable of being associated with, or could reasonably be linked, directly or indirectly, to you, when you: 

• Request a Callback via the Online Form. If you request to speak to a licensed agent, you will need to fill out the online form, which requires your full name, telephone number, email address, date of birth, state, and zip code. We use this information to communicate with your regarding your Medicare questions, provide support, or help you with a new Medicare plan.
• Communicate with Us. You may need to contact CVIG, but not a licensed agent. You can do this by email, telephone, or by stopping by our office. If you contact us by email, we will collect your email address and any other information, such as name or telephone number, you provide in the body of the email. We will never request, nor do we accept, your medical records, please do not provide them. If you contact us by telephone, we will collect your name, telephone number, and any other information related to your inquiry. Our Website uses Google Maps to provide you directions to our office. Once you click on the “Find us on Google Maps” button, you will automatically be redirected to Google Maps and be subject to Google’s Privacy Practices and Terms and Conditions. • Interact with Us on Social Media. When you interact with us on social networking websites, such as Meta, TikTok or LinkedIn, we collect basic engagement metrics and use it to tailor content and marketing and use it to improve user experience. Because our social media pages are publicly accessible, when you use them to interact with other users, for example by posting, leaving comments, liking, or sharing posts, any Personal Information you provide can be viewed by others or used by them as they see fit. The content your post on our pages can be deleted in the same way as other content you have created. We do not control the use or storage of information that you have posted to any social networking websites. This information is collected and processed by the social networking websites for their own purposes, including marketing. • Apply for a job. If you apply for a job, you will be redirected to our Careers website. You will provide us your name, email address, telephone number, physical address, resume, cover letter, and references. We use this information to communicate with you regarding the position, review your background and skills, and assess your qualifications.
• Aggregate data. We aggregate the data we collect for benchmarking purposes and for internal analytics.

PERSONAL INFORMATION WE COLLECT AUTOMATICALLY 

In addition to the Personal Information, you provide directly to us, we also collect information from you automatically as you use our Website via “cookies” and similar tracking technologies.  This includes the following internet or other electronic network activity information. 

We use essential, performance, marketing, and analytics cookies to collect your usage, device, and location information when you interact with the Services. We use this information to: (i) track you within the Services; (ii) enhance user experience; (iii) conduct analytics to improve the Services; (iv) prevent fraudulent use of the Services; (v) diagnosis and repair Services errors, and, in cases of abuse, track and mitigate the abuse; and (vi) market to you more effectively across different web pages and social media platforms based on your browsing habits and history. We do not use third party cookies on our Website. 

HOW WE SHARE YOUR PERSONAL INFORMATION

General Sharing 

CVIG shares Personal Information in the following instances:

• Within CVIG. We share your Personal Information among our affiliated entities for the legitimate business purposes of efficiently and effectively providing bonds to our customers. Access to your Personal Information is limited to those on a need-to-know basis.
• In the event of a corporate reorganization. In the event that we enter into, or intend to enter into, a transaction that alters the structure of our business, such as a reorganization, merger, acquisition, sale, joint venture, assignment, consolidation, transfer, change of control, or other disposition of all or any portion of our business, assets or stock, we would share Personal Information with third parties, including the buyer or target (and their agents and advisors) for the purpose of facilitating and completing the transaction. We will also share Personal Information with third parties if we undergo bankruptcy or liquidation, in the course of such proceedings.
• With Service Providers. We share your Personal Information with our service providers that help us carry out our business.
• With Third Parties. We may need to disclose your Personal Information to third parties, such as legal advisors, law enforcement agencies, or governmental/regulatory bodies to protect our legal interests and other rights, protect against fraud or other illegal activities, prevent harm, for risk management purposes, and to comply with our legal obligations. 
• For legal purposes. We share your Personal Information where we are legally required to do so, such as in response to court orders, law enforcement or legal process, including for national security purposes; to establish, protect, or exercise our legal rights, as required to enforce our terms of service or other contracts; to defend against legal claims or demands; to detect, investigate, prevent, or take action against illegal activities, fraud, or situations involving potential threats to the rights, property, or personal safety of any person; or to comply with the requirements of any applicable law.

HIPAA NOTICE OF PRIVACY PRACTICES

The Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) Privacy Rule allows individuals who provide protected health information to covered entities or business associate the right to receive a notice that describes how their protected health information may be used and/or disclosed and how to acquire access to this information. CVIG will never ask or accept medical records from you. However, you may provide certain healthcare information about yourself in order for us to assist you in applying for insurance coverage. We will only use your healthcare information for that purpose and no other purpose. We do not provide your healthcare information to third parties, except for the health insurance providers (or their agents) to whom you are applying. 

DO NOT TRACK

We do not respond to Do Not Track requests. Do Not Track is a preference you can set in your web browser to inform websites and mobile applications that you do not want to be tracked. You can enable or disable Do Not Track by visiting the Preferences or Settings page of your web browser. 

INFORMATION SECURITY 

We implement and maintain reasonable security measures, such as access controls and encryption, to protect the Personal Information that we collect and maintain from unauthorized access, destruction, use, modification, or disclosure. However, no security measure or modality of data transmission is 100% secure, and we are unable to guarantee the absolute security of the Personal Information we have collected from you. 

CHILDREN’S PRIVACY 

Our Website is not directed to or intended for children. We do not knowingly collect or post information from children under the age of 13 in compliance with laws protecting the privacy of children.

We may amend this Privacy Notice in our sole discretion at any time. If we do, we will post the changes to this page, and will indicate the date the changes go into effect.
